Client Resources & Consumer Information
In accordance with House Bill 4224 and Section 181.105 of the Texas Health and Safety Code, the following consumer resources are provided to support transparency and client access.
1. Request Your Health Care Records
You may request a copy of your therapy records at any time by contacting:

If applicable, fees associated with records requests are outlined in your informed consent document, which you can access anytime through your SimplePractice client portal.

It is clinically recommended that records are reviewed with the therapist prior to mailing to ensure clarity, safety, and appropriate clinical context.

Records requests will be processed in compliance with state and federal privacy laws.

It is clinically recommended, as a standard of care, that clients review their records with their therapist for clarity and supportive clinical context, though all record requests are fulfilled based on the client’s request in accordance with state and federal law. Signed informed consent forms—including any applicable records request fee information—are securely maintained and accessible to clients at any time through their client portal.
